This memo is for all Clinical Laboratories in region 3 who failed to participate in 2011 Serology NEQAS, who still intend to provide HIV/AIDS, Hepatitis B and C, and Syphilis serologic testing for CY 2012.
In order for you to include the said serology testing in your services you are hereby given until March 31, 2012 to register at NRL-SACCL and submit your Certificate of Participation at CHD3 for processing of your license.
Note: Acceptance of registration by the NRL-SACCL is from March to June 2012.
1. Accomplished application form provided by the NRL-SACCL/SLH;
2. Photocopy of renewed PRC ID;
3. Letter of endorsement from pathologist or head of the agency and certifying that he/she is a practicing medical technologist in the laboratory for at least 6 months;
4. Photocopy of renewed license to operate (or its receipt) a clinical laboratory and/or Blood Bank;
5. Training fee - Php 16,000 / participant

1. Accomplished application form for the renewal of HIV proficiency certificate provided by the NRL-SACCL/SLH and must be signed by the pathologist of the sending agency;
Photocopy of previous HIV proficiency certificate;
Quarterly census form for HIV and other blood-borne infections;
Certificate of performance signed by pathologists or chief medical technologists;
Certificate of training, seminar or convention (related to HIV and blood-borne infection);
2. 2 copies of 2x2 colored ID picture with white background;
3. Renewal fee (depending on the category of the applicant) to be paid at the San Lazaro Hospital.
Categories of Proficient Medical Technologist Requiring Renewal of Certificate
1. Category I - Medical technologists performing HIV test with proficiency certificate acquired after CY 2001 shall undergo training seminar update.
2. Category II - Medical technologists performing HIV test with proficiency certificate acquired before CY 2000 shall undergo refresher course.
3. Category III - Non-performing HIV proficient medical technologist and those with expired proficiency certificates shall undergo initial HIV proficiency training for reinstatement.

Under Administrative Order 2007-0027 section 6.B.f.ii, all clinical laboratories shall participate in an external quality assessment program administered by the designated National Reference Laboratories.
In this connection, all laboratories performing HIV/AIDS, Hepatitis B & C, and Syphilis serologic testing are hereby directed to strictly adhere to the schedule of activities and guidelines set by the National Reference Laboratory for HIV/AIDS, Hepatitis B & C, and other STIs for 2011 EQAP.
Refusal of the laboratory to participate in an EQAP conducted by the designated NRL will be a violation and will result to non-issuance of license to operate a clinical laboratory.
The schedule of activities in the conduct of NRL-SACCL 2011 EQAP is as follows:
January to June 30
Submission of registration form and payment
August 01
Distribution of panels for testing to participating laboratories
September 01
Submission of final results of laboratory participants to NRL-SACCL / DigitalPT
October 15 to November 15
Analysis of results
December 30
Dissemination of final report and results to participating laboratories
In order to observe the timeliness of the above activities, the said schedule will be strictly adhered to. Since there is 6-month ample time to register and subsequent activities are dependent on the number of registrants, late registrations submitted beyond the said deadline will not be accepted.
I happened to be selected as one of the participants in the training on HIV Proficiency that will be held at STI-AIDS Central Laboratory (SACCL), San Lazaro Compound, Manila.
This will be conducted by the Department of Health through the National AIDS STI Prevention and Control Program (NASPCP) which have funding support from the Global Fund Round 6 HIV Project. This is part of the strategy to continuously improve quality of health service delivery to Most at Risk Populations (MARP).

The hotel accommodations would be at Fersal Hotel (Andalucia - Fersal Hotel - Manila #1455 A. Mendoza Street corner Alvarez Street, Sta. Cruz, Manila, Philippines).
Arrival would be on January 23, 2011 and check-in at Fersal would be from 12 in the afternoon.
Transportation expenses (from home to airport to hotel, hotel to SACCL, from hotel to airport to home) should have proper documentation (travel voucher, receipt form, bus tickets, etc.) of exact value (as required by Commission on Audit). Non-compliance would be denied reimbursement.
To avail of land transportation reimbursement the terminal fee and boarding pass (Manila - Home flight) must be sent via courier (to serve as proof of travel).
Non-flight participants are allowed the weekends off, but travel expense will be shouldered by the participants.
Per diems one (1) day before and one (1) day after of participants and other incidental expenses shall be borne by the respective sending agency, subject to the usual accounting and auditing rules and regulations.
